Skilled Nursing

Our nurses work seamlessly with a patient’s doctor to develop
a plan of care that is tailored to meet the patient’s individualized needs.
Our nurses will…

  • Monitor a patient’s vital signs, medication and any special diet
  • Control the impact of acute and chronic illness such as diabetes and heart disease
  • Teach self-care techniques for asthma, diabetes, stroke, heart and respiratory conditions, wounds, ostomies and catheters
  • Arrange for additional services if needed
  • Communicate with others involved in the home health team
  • Help to reduce the complications caused by wounds and ostomies


Physical Therapy

After surgery or an extended illness, patients often require physical
therapy to help them regain their strength and mobility.  Following
a doctor’s order, our therapists are able to work with all different
treatment modalities.  The goal is to achieve the patient’s maximum potential of independence.

Our physical therapists will…

  • Provide ultrasound treatment when needed
  • Assess home safety
  • Offer instruction in the proper use of walkers
  • Teach transfers from bed to chair, tub, toilet and or wheel chair
  • Create individual exercise programs that help the patient with balance, pain management, education and fall prevention

Occupational Therapy

Some patients require occupational therapy to improve their function of tasks such as dressing, bathing or eating after an injury or illness.  They also work with upper extremity injuries to maximize independence and cognitive rehabilitation.

Our occupational therapists will…

  • Assess your home for safety such as poor lighting, tripping hazards, clutter, handrails, electrical cords, furniture, etc.
  • Help to set up your kitchen with attention to storage and accessibility
  • Access bathroom needs with regard to grab bars, shower seat, etc.
  • Maximize your independence and key into fall prevention

Speech Therapy

After surgery or stroke traumas, patients may need therapy to facilitate communication or deal with swallowing disorders. Often it involves the retraining of patients in breathing, swallowing and muscle control. Our speech therapists are pro-active in training family members as well.

Our speech therapists will…

  • Assess baseline neuro-cognitive ability for immediate and recent memory, problem solving and abstract reasoning
  • Help with safety awareness in the home environment
  • Treat aphasia (difficulty in speaking)
  • Train family on cueing systems
  • Provide strategies to aid in word find
  • Help with interpretative training of maps, prescriptions, appointment cards, etc. Provide voice training to both patient and family
  • Provide memory and naming techniques

Social Workers

A medical social worker evaluates the social and emotional needs to
help a patient thrive at home.  A qualified social worker offers counseling to patient and family members.

Our social workers will…

  • Find the resources available in the community at large
  • Assess the relationship of the patient’s medical and home situation
  • Assess the social and emotional factors related to the patient’s illness
  • Coordinate the Medicaid conversion
